Have Questions?

What's included in the Module 4 Documentation Package?

The Module 4 Documentation Package includes 15+ audit-ready policies, procedures, forms, and registers covering specialist support coordination (Level 3) under registration group 0132, aligned to NDIS Practice Standards for Module 4. You also get implementation resources (user manual, pre-filled samples, self-assessment answer templates), an audit preparation manual, folder structure template, two bonus resource packs (NDIS Information Pack and Audit Preparation Guide), and 3 months of Provider Hub access for ongoing doc updates, compliance resources, and regulatory alerts.

Specialist Support Coordination, also known as Level 3 Support Coordination, is a higher level of support for NDIS participants with complex needs, high-level risks, and significant barriers to implementing their plan. It focuses on reducing complexity in the participant’s support environment, resolving urgent or complex issues, and helping the participant connect with appropriate supports and services. This may include understanding and implementing the NDIS plan, designing support approaches, establishing and coordinating services, providing targeted support for specific challenges, crisis planning and prevention, addressing barriers to plan implementation, building the participant’s capacity and resilience, and reporting progress and outcomes to the NDIA.

Specialist Support Coordination, also known as Level 3 Support Coordination, must be delivered by an appropriately qualified and experienced practitioner using an expert or specialist approach. Eligible practitioners may include psychologists, occupational therapists, registered nurses, mental health nurses, social workers, counsellors, speech pathologists, developmental educators or other allied health professionals with relevant qualifications, professional registration or membership, and demonstrated experience supporting participants with high-complex needs.

NDIS Support Coordination has three levels, each with a different scope and level of intensity. Level 1, Support Connection, provides time-limited support to help participants connect with informal, community and funded supports, and is the most basic level of support coordination. Level 2, Coordination of Supports, helps participants understand and use their NDIS plan, connect with appropriate supports and build their capacity to manage services more independently. Level 3, Specialist Support Coordination, is a higher level of support for participants with complex needs, high-level risks or significant barriers to plan implementation, and must be delivered by appropriately qualified practitioners. Each level is linked to different NDIS registration groups: Levels 1 and 2 sit under registration group 0106 and are covered by our Support Coordination Documentation Package, while Level 3 sits under registration group 0132 and is covered by this Module 4 Package.

Yes. Module 4 is a Supplementary Module, so you’ll also need Core Module documentation to complete your registration and audit requirements. If you need documents only, consider the NDIS Certification Documentation Package. If you want full support, consider the NDIS Certification Platinum Package. If you need two or more Supplementary Modules, the NDIS Mega Platinum Package is usually the most cost-effective option.

The two packages cover different levels of Support Coordination. The Support Coordination Documentation Package covers Level 1, Support Connection, and Level 2, Coordination of Supports, under registration group 0106. The NDIS Module 4 Documentation Package covers Level 3, Specialist Support Coordination, under registration group 0132. Some providers only deliver Levels 1 and 2, which is the most common pathway, while some providers deliver Level 3 only, usually where specialist clinical expertise is involved. Many providers who deliver Level 3 also deliver Levels 1 and 2, meaning they typically need both packages. If you are registering for 0106 only, you need the Support Coordination Documentation Package. If you are registering for 0132 only, you need the Module 4 Documentation Package. If you are registering for both 0106 and 0132, you need both packages, or the Mega Package, which includes all modules.

Instantly. After completing checkout, you’ll receive a download link immediately via email and on the order confirmation page. All files are included in a single download.

Plan for 12+ hours of work on average — faster if you can dedicate focused time. The included user manual guides you step by step.

No. This is a DIY documentation package. You handle the application, auditor selection, and audit preparation yourself. We provide self-assessment answer templates to guide you, but you submit the application. If you need full support, see our Platinum Package.

Our 24-hour Missing Document Guarantee covers this. If your auditor requests a policy, procedure, form, or register within your Module 4 package scope that we didn’t include, we’ll provide it within 24 business hours at no extra cost. Terms and conditions apply.

Yes. Add the NDIS Documentation Editing Service to your order. Our team will edit and brand your Module 4 documentation with your business name, logo, and key details — delivered within 24 business hours. This is an add-on service separate from the Module 4 Documentation Package itself.

Templates is a DIY documentation package — you customise the documents, submit your own application, find your own auditor, and prepare for audit yourself. Platinum is a done-for-you system that includes branded documentation in 24 hours, full application submission, full audit support, a dedicated team member, 12-month Provider Hub access, and a money-back registration guarantee. There is no standalone Module 4 Platinum tier — the only Platinum package covering Module 4 is the Mega Platinum Package, which also covers the Core Module and all other Supplementary Modules.

Yes. This package can be used to support a scope extension or variation of registration for Module 4: Specialist Support Coordination under registration group 0132. If you are already registered for the Core Module and want to add Module 4 to your registration, you can use this documentation to help prepare for the scope extension application and audit requirements.

No. Due to intellectual property protection, template purchases are final and non-refundable. Make sure you choose the right scope first. See our Terms and Conditions.
